To help ensure our staff can process your information and get you started with your care on time, please come prepared with the following documentation.
Documents to Bring to Your Appointment
Proof of address (i.e., current utility bill, bank statement, other U.S. mail)
Photo Identification (any one (1) of the following documents: passport, driver’s license, state ID)
Social security number, if applicable
List of your medications (including any you need refilled)
List of any known allergies to medication, food
Insurance cards, if applicable (primary and secondary, if applicable)
Proof of income. Why do I need this?
Payment for services rendered. We accept checks, cash, debit/credit cards and/or money orders.
